What a note includes
Name, email, your note, the time of the request, and a user-agent. That record is stored so the practice can reply. It is kept for a limited period and is not published.
What this is not
Not a medical record, not a HIPAA patient portal, and not informed consent for treatment. Do not send what you would only share inside a clinical relationship.
