This site is a conversation door, not a clinic portal.
What a note includes
Name, email, your note, the time of the request, and a user-agent. That record is stored so the practice can reply. It is kept for a limited period and is not published.
What this is not
Not a medical record, not a HIPAA patient portal, and not informed consent for treatment. Do not send what you would only share inside a clinical relationship.
Treatment records
If we agree to work together, a privacy notice for that clinical work is provided in writing before ongoing work begins. This website form is not that notice and does not create a medical record.
Measurement
A third-party analytics script loads only when a measurement ID is configured on this site. Page views, CTA clicks, and a note-sent event may be recorded. No clinical content is sent.
